Name and address of establishment

Name and address of principal employer

Duration of contract: From

Number of days during the half-year on which-

The establishment of the principal employer had worked

The contractor's establishment had worked

Maximum number of contract labour employed on any day during the half year:

Men

hours of work and spread- over

Whether weekly holidays observed and on what day

If, so, whether it was paid

Number of man-hours of overtime worked

Number of man-days worked by

And at last along with the date and place the signature of the contractor has also been attached
