Form 9 Application For Restoring The Name In The Register


 Restoration of Name of Member 

Eligibility

A member whose name has been removed from the Register of Members is eligible for Restoration of Name

Requirements:

The application in Form 9 duly filled in and signed by the member along with the following fees. In case the member has changed his signatures, the new signature verified by a chartered accountant may also be provided.

a. Membership fee for the year in which the name was removed.
b. Membership fee for the year in which restoration is sought.
c. Certificate of Practice fee (if intends to hold).

The name of the member will be restored from the date of receipt of Form 9 together with requisite fee and compliance of all the requirements.

The name of a member can be restored with retrospective effect (i.e. from the date of removal) provided an application for restoration in Form 9 along with membership fee, Certificate of Practice fee (if intends to hold) and restoration fee is received within the relevant financial i.e latest by 31st March.

 Removal due to the following reasons:

1. Non-payment fee within the stipulated time.
2. Own request –A Member has to pay the fee for the relevant year and mention the date of removal for updation in the records.
3. Death- On receipt of death certificate the removal updation is done.
