There is also a declaration as per which it is certified that 

(i) that  ** I/we have paid Bill No………….of………..[Name of the hotel dated …….on behalf of foreign tourist(s) in Indian currency;

that the aforesaid payment has been made out of Indian currency obtained by conversion of foreign exchange received in, or brought into, India through an authorised dealer;

(1) Reference/name of the group/traveler……..

(2) Name of the foreign agent, i.e., remitter  

(3) Name and address of the authorised dealer         

(4) Date(s) of receipt of foreign exchange by the authorised dealer 

(5) Number, date and the amount of the bill of claim of the travel agent/airline on the overseas 

And at last there is attached the signature of the person designated
