Application For The Issue Of Certificate Of Identity

 

This application must be accompanied by three passport size photographs one duly attested by local police authority as well as a residential permit. It is an application seeking the issue of certificate of identity. First and foremost, the amount, date of payment as well as mode of payment are the particulars required for this application.

 

Subsequently, personal particulars such as name, aliases (if any) of te person is sought, including any previous name in case the name has been changed in the lifetime off a person.

Maiden name is also required in case of a married, divorced or a widowed woman. The following are general particulars required:

10.  Date of birth.

11.  Height.

12.  Colour of eyes.

13.  Colour of hair.

14.  Visible distinguishing marks, if any.

 

The form also reuqires particulars of child/children below 15 years of age, to be included in the certificate of identity.

 

Thereafter, purpose of applying for the certificate of identity as well as the countries for which the certificate of identity is required concluding with the declaration of applicant and his signature.
